Evaluation and Management Coding

Our focus and field of expertise is with all aspects of Evaluation and Management Code assignments. Evaluation and Management CPT codes are codes used by a physician to report evaluation and management services including but not limited to patient history, a focused exam and or medical decision-making. These services may be provided in an inpatient and outpatient setting or other ambulatory facility.
Evaluation and Management Services are divided into a broad set of categories and subcategories (e.g., outpatient services, inpatient services). Medical documentation for Evaluation and Management Services must consist of the appropriate components (e.g., history, physical exam, medical decision making) as designated in the 1995 and 1997 Physician Evaluation and Management guidelines as published by the Center for Medicare and Medicaid Services and Current Procedural Terminology (CPT) manual.
